I am out of medication, how do I get a prescription refill?

The first step to refilling your prescription is to call your pharmacy and request a refill request to be faxed to our office. To learn more about our prescription policy, please click here.

How do I get a copy of my medical records?

Most importantly we would need a signed release then a required minimum $10 fee must be paid.

Do I need a referral to come to your office?

To see a physician it is based upon your insurance coverage whether or not you will need a referral. If you are a physical therapy patient you do not need a referral.

I am a veteran, what steps do I need to take to become a patient of your office?

If you are a veteran in need of orthopedic care, contact the VA for a referral to our office for an appointment.

Can I make an appointment the same day I call your office?

Yes, Guilford Orthopaedic and Sports Medicine Center proudly accepts same day appointments. To make an appointment today please call 336-275-3325.

What if I don’t have insurance?

We accept self-pay patients; a $300 deposit is due at the time of your appointment.

How much is my copay?

Your copay amount is listed on your insurance card next to where specialist is listed.

I was injured on the job, what do I need to do to make an appointment?

If you were injured on the job and need to make an appointment with us, contact your employer to have them schedule your appointment.

I was involved in a Motor Vehicle Accident (MVA), what do I need to do to make an appointment?

We do not involve ourselves with third party cases, in which you would set up your appointment and file your personal insurance.

No, we do not currently provide transportation to our facility.

After Hours Urgent Care:
Monday-Friday: 5:30 pm to 9:00 pm
Saturday: 9:00 am to 2:00 pm
Sunday: 10:00 am to 2:00 pm
